Currently I can add the form to the master ticket but not the subtask. The purpose for this is that I am trying to add a checklist or checkbox field to subtask. The master ticket will have multiple subtasks and each subtask will have a different type of checklist or check boxes.

I can suggest various add-ons developed by my team, as I am unsure about your specific usage scenario. However, in my opinion, the
Recurring Checklist and Reports
add-on which may help with your needs. With this add-on, you can create checklists in two ways - the first is to create recurring task with a checklist inside, and the second way is to add a checklist to an already created task with one click.
With the ability to reporting in this add-on, you can also check report for the submission dates of all your employees and information from the checklist in one place.
You may also try to check Smart Jira Forms functionality cause you mention that you need a Form or Checklist. In this add-on you can add a form to issues automatically or manually, when the respondent submits the form, you can see it inside issue, inside add-in reports or export responses in excel. Our new feature destination issue fields will auto-populte jira issue fields based on form responses if you will add a form inside an issue. Plus there are a lot of other functionality like - sharing forms externally, create issue after form submissions and conditional logic.
If you have more complex scenario you may wanna try Business Process Manager add-on. With this add-on you can set form or checklist with workflow transition after submission or approval workflow to build multi-step processes. Let me know if you need further assistance!

Or you are trying to work with Jira custom fields?
I can recommend you use our Issue Checklist for Jira Free which allows you to add checklists automatically to tasks, and subtasks using either automation, default checklist templates, or label integration.

If you make a template in the checklist you can then add that to a sub-task when it creates it will add the checklist.

Are you using issue checklist for Jira?
If so you can go into checklist managed template and create a template and save it. Once you save that checklist template you can then add it to default to add that template to sub-task by manage checklist. Add default issue type(sub-task) to checklist.
This will populate the checklist you are requiring in each sub-task.
