Remove the "Create this request on behalf of" on the portal support

Hello, 

I want to remove the "Create this request on behalf of" on the top of the request form on the portal, do you know how to remove it ?

Also, I'm wondering if it's possible to remove the field resume when I working on the request form because I only want that the user can see the form that I create from the section Form

The "Create this request on. behalf of" cannot be removed, that is the reporter field and is used for customer notifications among other things.

Yes, you should be able to remove the resume field, the only field that has to be on the request form is Summary, but you can always hide it and give it a default value. That is what I do on request types that we only are using a form on as the intake, I can then use automation to update the summary field is needed with data from the form.
