Have you checked the Customer access permissions? Perhaps a new user tries to enter the form and is not allowed to create an account
We have disabled the feature but if you check "Customers can access and send requests from the portal without logging in", then everyone should be able to send in the request forms.
Also, there is a warning in your environment about configuration problems (as shown by the pop-up). Make sure that you those problems don't interfere with your workflow.
