Using the service management free plan and disabled email notifications years ago but just started getting this popup. What notifications are being sent and now I'm worried the system is sending out emails to clients or someone without us knowing.

How exactly did you disable the email notifications? Have you rechecked to confirm they are still disabled?

Jira projects use notification schemes to send email notifications about project activity. There are two ways to disable email notifications for a given project:
Remove the notification scheme entirely from a project.
Edit a project's notification scheme so that no emails are sent. Take care when editing a scheme as more than one project may be using it.
Are you sure that notifications are disabled for all projects? Perhaps a new project has been created.
The limit applies to all emails that are sent from the Jira system. Do you have any Automation Rules that are sending emails?
Under jira settings, system, outgoing mail. It's showing disabled system wide so it shouldn't be able to send even if enabled anywhere, including automation. I'm wondering if notifications include things other than email like api integrations or something as we have teams notifications and some automations

Per this document the Outgoing Email setting does not impact sending emails from Automation Rules.
Automation Send Email action
When an Automation rule is set to send an email, even with the Outgoing Mail disabled, the notification will be sent since the "from" email address is <no-reply@automation.atlassian.com>.
