Automation Pro Rule creating multiple Request

Hi All,

 

I'm currently trying to create an automated task to run on a monthly  basis to create a service request and be assigned to a queue. Currently the Rule work correctly but three request are created not just one and not sure why it is happening. 

Screenshots attached of how the rule is configured, any feedback would be greatly appreciated

Update:

 If I remove the JQL filter (Project = "Our Servicedesk Project"only one issue is created but then I receive the attached error
