We are using Jira cloud version and want to know what extension are available for customizing "my request" view for customer portal.
We want status drop down to have more statuses linked to actual jira workflow and not just Open,close and Any status.
Also, add few more column like Created, Updated date etc. Any, working extension details would be highly appreciated as we have many customer on our service desk and they all think its a limitation with which we agree too!

Currently, it's not possible to customize the customer portal to add more columns or status.
We have some feature requests suggesting the implementation if this ability:
- Allow customization of customer portal to be able to include more rows
- Change columns in Customer Portal
- Need more fields in the 'My Requests' view

I've checked the add-ons and the one that seems to offer more customization of the customer portal is Extension for Jira Service Desk.
As the one that you found is only for server, you can check their contacts on the marketplace and send an email or contact their support to check if they will develop the same add-on for cloud.

This is something that has come up for our users as well (we just implemented JSD). They would like to see the creation date of issues when looking at the "My Request View".
Any updates on if this is something that might be coming in the future?

Currently the only option is using app, like the one we created - Advanced Portal Reports. Feel free to try it.

Atlassian added a simple option to the native Requests page. You can read about it here. It's a basic.
For more demanding customers, we've recently released My Requests Extension for Jira Service Management. I'm a product owner of this app. It provides a dedicated Requests view which allows to display more information to your customers. List of all supported fields you can find in our documentation. This list is still expanding! Re-arrange columns, sorting, exporting etc. are available. We're working also on filters functionality (the same like in the server version).
