Hello!
I'm working on an issue type where we have a dependent dropdown menu with two options: Yes or No. I'd need to have a dependent option being triggered when we select "Yes" where we would be able to enter the name of a person.
Is it that the dependent menus only work with pre-selected fields? Would there be a way for us to have this option to have the reporter add some information when selecting Yes on the main question, as opposed to having an optional field below on the request form?

So neither Jira's issue create screens nor JSM's requests have the ability to create dependent or conditional fields like what you're talking about.
BUT! Atlassian recently acquired ProForma which does have the ability to have sections with required fields appear based on other fields:
Now that it is "built-in", these Forms are only available in Jira Service Management, but ProForma used to support forms in Jira Software as well so the rumor is that if you ask Support, they can enable it for your software projects.
