Install both on the same Jira Core server (or add one to an installation of the other), via Manage Applications. They "integrate" automatically then.

If you are talking about Atlassian cloud, don't worry. Atlassian will take care of the integration.
If your question is about a third party cloud service, you'd install JSD along with Jira Software, use the same port during installation and they are integrated automatically. No plugins or applications are required.
While doing JSD installation after Jira Software installation, if you specify different ports for both, they'd be different instances on the same server and hence not integrated.

Both applications are natively integrated you only need to install both on the same server.

Currently we are using JIRA cloud at one isntance and Jira Service desk at other instance

No, you don't need to install anything the APP's are totally integrated by default.
