Make custom fields

This question is in reference to Atlassian Documentation: Configuring a Custom Field

how to make two different custom fields. One is single selection and another one is text writer field. The both fields are show in one custom field eg. i have attached the sample picture Please do needful for me 11.jpg

Those are two separate fields, not one.

The best you can do is make the appearance of one field dependent on the other.  Try the "behaviours" functions in the Script runner add-on, that's the easiest way to do it.
