Power up to create a form linked to a board?

Hello,

I have a question about the Power-ups. Is there a power up that allows me to design a form to create a Trello card?
Here is my situation: at my company, we have a "design request board" on Trello, and I would like to create a form so my colleagues can create a card following the informations needed, like "description of the request", "deadline", "Inspiration", "examples",... exactly like this form (this one is linked to Jira): https://form.asana.com/?hash=f5e7888e57b105de1881f4766380137d36e3d0b2ec908928bf95bb0dac5d0a49&id=1149336594301957

Toni here to help! Maybe Crmble Power-Up  which allow you to create automatically Trello cards every time a new Google form or Facebook Lead form is fulfilled. It could work for you?
