How can we perform Agile Maturity Assessment survey using Confluence?

We want to have an Agile Maturity Assessment survey done where users can provide responses(1(lowest) to 5(Highest)) for each questions and responses should be saved and presented in the form of matrix.

How can we have this functionality in Confluence?

Natively Confluence does not have any functionality for a survey. This can be achieved using different plug-ins, results of which can be collated onto a Confluence page. I would suggest you evaluate the available plugins on the marketplace to identify the one best suited to the use case.

I have used one called SurveyMonkey for Confluence (Forms & Survey) and it provides a variety of options for capturing user responses.
