We're trying to create a form that will send out a notification if a specific checkbox or radio button is selected.
I have the form filling the field out in a new issue just fine.
I can't figure out how to attach the completed form to the issue or notification so that the receiving department can see the relevant information without needing access to the project from which the form was submitted.
Edit: I'm looking to automate a new issue or email notification with the completed form attached.
@Sam Greilick If you were to tie specific form fields to custom fields you could use automation to create a new issue and use the automation to place the custom field values into the description of the new issue or into the same custom fields.

This is the same process I'm using to automate notification sending only if it's pertinent to a department, so it shouldn't be to hard to just extend it to the relevant fields.
