Mobile Device Agreement Form in JSM

Hi everyone.

I am trying to build out a Mobile Device Agreement form in JSM.

The agent will fill out part of this form(Device Model, Serial Number...) and send this form out to the customers(internal employees who don't have an active Jira license. JSM customers) so they can fill out their name, email address, and consent to the terms and agreement of using company-owned devices.

Ideally, I want to have one PDF document generated with the Device information, User information, time date, and their consent/acknowledgment of the company terms (audit purposes). 

Does anyone have a suggestion on how I could achieve this? Many thanks.
