Using proforma forms or the new JIRA forms, I need to set up

Hard to illustrate, but they would be checkboxes and then topic c would have sub choices. Is there a way to do that?

Currently, Proforma form doesn't support Cascade-select field (Jira/JSM). See the enhancement request currently being tracked by Atlassian - https://jira.atlassian.com/browse/JSDCLOUD-10792
However, take a look at Proforma documentation on using conditional logic / automation rule to overcome this limitation (I am assume that you want to push the data captured in the Form back to the Jira/JSM issues and not just serve as an attachment to the issue) In this example, you actually will create a custom field with data type of "cascaded-select-list" for your Jira/JSM project, then use the workaround below as suggested in the link below to push the data captured in the Form back to the custom "cascaded-select-list" field associated to the issue.
