So, I have this automation where if there is no sub-tasks present then it will create a sub-task TEST. But I need help with, how can we create a sub-tasks "TEST" once even though there are multiple sub-tasks present. And how can we avoid making duplicate sub-task if the status changes from Done to To-Do and again from To-Do to Done

You can augment condition where you are checking for Subtasks to ensure there are none with "TEST" (or whatever key word appears in your subtask summary) are present.
Try the following:
You'll need to update the JQL to search for the summary of the subtask you want it to create.
