Welcome to the Atlassian community.
When filling out the support request, make sure that you click on Data Center on the right when selecting a product.
Then you will get a prompt to enter your SEN.
The prompt to enter your URL would apply only if you have selected a Cloud product.
This is a publicly accessible user forum. Don't paste SEN/License keys here. I removed it from your post.
