Hey Dkwan,
Unfortunately, we don’t have a timeframe for when this integration will be released. But for more information on the new availabilities of Jira Forms for JSM, JWM, and JSW products, we will be updating our users via email and also our roadmap here: Atlassian cloud roadmap
Allow me to share with you the feature request that we have related to this topic:
Should you have any concerns, kindly let me know!
Thanks,
Hello Dkwan,
At the moment you can use Advanced Forms to have conditional section fields in Jira Work Management(JWM). So if you are still using the ProForma Marketplace app in JWM, you’ll see an option for Advanced Forms in project settings. If this applies to you, you can learn more about Advanced forms.
Allow me to share with you an excerpt from this documentation:
- To group fields into sections, select Section, and drag and drop a section element above (and another section element underneath) your desired field groups.
- To make a section appear conditionally based on the data entered in a previous field, select the section above the fields you want to appear conditionally, and enter the conditions in the panel that appears on the right.
- For a section to be conditional, the preceding field must be a radio, checkbox, or dropdown field.
If isn’t apply to you, as Advanced Forms aren’t officially released to JWM at the moment, we recommend you try out some third-party apps available in the Atlassian Marketplace that might help you to achieve your goal.
I've taken a look at our marketplace and I found these add-ons:
I hope that this information helps and let me know if you have any questions, happy to answer them!

Thank you. I don't think we are on advanced forms right now. Do you know when it will be released to JWM?
I looked at Dynamic forms and it seems like I can only customize the screens for creating issues, not a form in JWM.

Late but wanted to update as i was also looking for this and found that there is progress :)
Conditional logic in Jira forms enables specific questions to be displayed or hidden, based on the answer selected in previous questions. This helps capture the right information when a form is being submitted and also makes completing a form easier and more efficient. Conditional logic will be available for selected fields and will be available for forms in business projects and software projects (once forms are available here).
