Can a form template be set up with SLAs, embedded so that when we add the due date/start date it fills in the rest of the dates?

You use third-party add-ons for the SLA functionality in JSW.
For instance, you can use SLA Time and Report for Jira Cloud, which includes some automation. By setting Start/Pause/Stop conditions, you can use different triggers (time limit until the first response, for example) to change the status of the problem or change the assignee, send notifications, and so on.
The add-on has a 30-day trial, so you can check if this solution suits you. My team develops the application, so if you need further assistance, please let me know.

@Dave Mathijs , like the first response date, status change date etc. This is not a JSM project that is why I am trying to come up with a solutions, if possible. Thank you.

You can create custom date picker fields for your Jira Software project, but you won't be able to use the SLA functionality in JSW.
